How to ensure PDF forms are filled in correctly
You must download PDF forms to your computer (or other device) using the “save as” function. This will allow you to complete the form properly.
Opening and saving PDF forms within your browser will likely cause functionality issues or loss of data.

When providing documents to the Immigration Appeal Division (IAD), appellants or their counsel are required to provide a written statement indicating how and when they were provided to the Minister's counsel.
If a party does not want to provide documents in support of their appeal, they must tell the
IAD in writing.
You can use the
Notice of disclosure form (694
KB
PDF) to provide your written statement.
Once completed, the form must be submitted to the
IAD and Minister's counsel along with the documents being disclosed in the appeal.
If more than one document is being provided,
a list of documents must be submitted together with the documents.
